If you use Qualtrics, carefully consider the risk level of your survey.
Surveys used for research should continue to follow the IRB approval process. If your survey is NOT
part of an
IRB-approved study, please refer to the risk levels below for direction. Violation of this process could lead to
deactivation of your survey, remediation efforts, and possible discipline. To see the data classifications linked
below you will need to be on the Jefferson network.

Survey questions and/or responses that are likely to include Protected Health Information (PHI) or
Personally Identifiable Information (PII)—see data
classification for details. |

| Survey questions and/or responses that are likely to include sensitive corporate data (i.e.,
compensation information, Jefferson Strategic Initiatives)—see data

| Moderate Risk |
Survey questions and/or responses that may include any other sensitive data, (for example, you might
ask about employee retention, market variables, or management styles)—see
data classification for details. (Note: Any potential for PHI and PII should always be high risk)
